The Aging and
Disability Resource Center, or ADRC, serves the elderly and adults with
disabilities. The ADRC also provides support and assistance to the families
and caregivers of the individuals we serve.
Eligible for services:
·People age 60 and older
·People (age 18 and older) with
physical or developmental disabilities
·People (age 18 and older) with
mental health or substance abuse problems
Services:
·Information and Assistance
This is a free and confidential service. Our
trained information and assistance specialists are knowledgeable about all
of the programs and services available to help meet the needs of older
people and people with disabilities. Contact us if you have questions or
want information on these or other programs and services you may need.
·Assistance in accessing
benefit programs: Medicare, Medicaid, Social Security, and others
Contact us when you have questions about
public or private benefit programs or other legal issues. Our Benefits
Specialists can talk with you about the benefit programs available and help
you apply for benefits (such as Social Security, Medicare, Medicaid, and
others).
·Transportation Services
The ADRC can help provide you with
transportation for doctor visits and many other activities. For scheduling

·Nutrition Programs
Good Nutrition is an important part of
maintaining good health. Getting together at a meal site or receiving a
home-delivered meal makes getting a well-balanced meal easy. Contact the
ADRC to learn more about the nutrition
services available to you.
If you have questions about your options for
long-term care, we want you to have all the information to make your own
choices. Contact us for objective, in-depth information about the services
and resources available to meet your current and future needs. We’ll
discuss with you the types and costs of care available, whether receiving
services at home or in another community setting.
·Programs for your health
Staying active and healthy is key to
maintaining your independence. We offer referrals to several education and
prevention programs and to screening services to help protect your health
such as blood pressure screening, memory screening, short-term medication
management, health education with one-on-one consultations, home safety
assessments, and programs to prevent falls.
·Support for caregivers
Caring for someone who is elderly or disabled
can be a rewarding experience, but it can also present challenges. We offer
information, support, and assistance to help family caregivers care for
their loved one and themselves. We can also offer support to grandparents
raising grandchildren. Contact us to learn more about our local Caregiver
Support Group, Annual Caregiver Day, Low-vision Support Group, Alzheimer’s
Support Group, and respite opportunities.
·Help transitioning from
child to adult services
Change can be challenging. We can talk with
and provide assistance to teens with disabilities and their families as they
transition from children’s programming to services for adults.

If you are on Medicare, the following information
may help you with medical costs. The Medicare Savings Program helps
people pay the cost of their Medicare premiums, co-payments, and
deductibles. Anyone who is eligible for this program will automatically be
eligible for help with their Medicare Part D prescription
costs. Program eligibility is based on your income and assets. This year
changes in federal law mean that more people will be eligible for this
program. Contact the ADRC to determine if you are eligible for the Medicare
Savings Program or for help applying.
